Did you add all key's to the same account?
If you created seperate account's for each key, your only option is to contact support (www.guildwars.com/support/) and hope that they will combine the accounts together - they do this by closing the excess account's and issue'ing you with new cd-key's... so you will loose whatever progress you have in those chapter's. If you added the key's into 1 account then you need to do special quest's to unlock travel to the other contenant's - these are found in Lion's Arch in Tyria, Kaining Centre in Faction's & Kamadan in NightFall. If all access key's are in the same account, any character you create from each chapter will be able to travel to the other chapter's.. once you've completed the respective quest's.

Your first assumption was correct, you can play all the games with the same characters.
With all the games you have 3 starting points 1 for each game, each has a point where you reach the main harbor and can cross to the other games. For the first game prophesies its called Lions Arch and you get there by following the main storyline accross the mountains to the west. For the others its when you leave the starting island or very soon after. The first 3 games then have another crossover point to the GWEN addon, which is for 20th level characters only.
